I understand that by checking this box I agree to the following:

1. Handheld launchers, Bird Bangers and Screamer Sirens are for use in wildlife pest control ONLY.

2. Instructions for proper use come with each product. In these instructions, Vegetable Growers Supply. and Reed-Joseph International Company recommend the use of ear and eye protection with all pyrotechnics.

3. All pyrotechnics should be stored to prevent access by children or unauthorized persons, and they should be stored in a cool, dry, non-residential location. Pyrotechnics should be stored in original packaging, and inside a locked metal container with a non-sparking interior.

4. Vegetable Growers Supply and Reed-Joseph International Company shall not be held responsible for any injury resulting from the use or handling of these devices, whether or not used in accordance with the directions.

5. The buyer agrees to secure local and state use permits as required.

6. The buyer of explosive products must be 21 years of age or older.

7. The buyer hereby accepts said materials subject to all terms hereof.

Please be advised that all Vegetable Growers Supply sales records for pyrotechnics and related shipping documents are available to and are subject to examination and review by Bureau of Alcohol, Tobacco, and Firearms and Explosives – US Department of Justice and US Department of Transportation.

I herby certify that I have a bona fide bird and/or wildlife control use for these pyrotechnic devices and certify that they will be used ONLY for bird and animal control purposes. I further understand that any other use of these items is STRICTLY PROHIBITED BY LAW. I further understand that by my signature I am certifying, under penalty of perjury, that all statements given by me in this Wildlife Control Statement are factual and true.
